I suspect there are many more similar patients whose correct diagnosis is delayed or missed, who could benefit from ECT.
My colleagues and  reported a similar patient in JECT in 2009:
  Successful electroconvulsive therapy in a patient with intellectual disability and bipolar disorder, with catatonic features misdiagnosed as encephalopathy
