Here's a nicely presented case report, first-authored by a Cornell medical student. I recall treating a very similar patient  a few years ago.
The only only issue I would raise is the choice of electrode placement, in a patient in whom there are medical safety concerns; bilateral electrode placement might have been faster and more effective for the depressive symptoms in this patient.
